Slope Intercept Form: y = mx + b

m = slope
b = y intercept

x₁ = 2
y₁ = 5

Point Slope Form: y - y₁ = m(x - x₁)

y - 5 = 4(x - 2)
   y - 5 = 4x - 8
     + 5        + 5
---------------------
   y = 4x - 3
